TRT Bloodwork

Hello All.
I went on prescribed TRT a month ago (my urologist, not an online clinic). Doc put me on 200 mg Cyp. I just got one month blood:
Total: 1238
Free: 9.2
Estradiol: 78 !!!!!
Hematocrit/Hemoglobin/Liver: All good
Before the Test #'s came in I asked for Adex since I'm holding a ton of water (puffy ankles). He prescribed: 1 mg M/W/F (seems really high). Then, the PA called saying reduce the shot to .8 ml (160mg).
I'm curious what you guys think. Reducing the shot might put me at 1000 or so that they want but I'm mostly worried that my Free is only 9 and may drop below "normal" range?
For now I'm following to the letter. It took over a year to get approved and picking up my pharma test covered by insurance is awesome and don't wanna blow it. That said, I'd love to have some feedback.

If you're pinning once a week, the fluctuations on test levels may cause more E2. Try pinning 3x or everyday for more stable levels.
I agree, at the very least do your shots every 3.5 days (e.g. Sunday AM, Wednesday PM), that will help a ton. You really don't want to crater your e2, it feels like absolute shit.

I'll give this a try. I have blood in 3 wks so I'll find out then. Any thoughts on my Free Test being only 9? I'm trying to understand how my body reacts to Free vs Total?

Your body seems to be responding well to that dose. Most start at 150mg and titrate up as needed. You may only need 75mg E3D.

They reduced my Adex but I'm waiting to hear what they want about the shot. I'm worried their idea of "Normal" is far less than mine.
This E2 crash needs corrected bad....

🤦‍♂️ 3 mg of adex per week. That’s why your e2 got pummeled. Try 0.5 mg twice a week.

78 e2 for 1200 testosterone isn’t all that much higher than normal.

Exactly... Up to this point I was trying to just follow instructions exactly, even though I have a few years experience on my own.
I'm down to 1 mg/wk. I just hope the doc leaves my T dose alone at this level. They seem really conservative. I can always just continue but wait a little longer after the shot to give the blood.
Any thoughts on how to boost my Free numbers without cranking up the total? Clomid maybe?

Don’t take clomid.

You need a new doctor. You don’t know enough and your current doctor must be a retard.

There’s so much wrong in this. Level with us, there is no doctor is there?

There absolutely is a Urologist. These updates and messages are coming from the PA. I messaged today asking to at least tell me their desired levels instead of telling me lower.
I've mentioned twice about my free being on the low end and not wanting to reduce anymore.
She ordered blood for the third month in a row telling me to reduce Adex to 1 mg and maybe that will pull things in line.
I haven't asked them but I'm wondering if splitting the dose would give any noticable improvement to both Estradiol and Free?

I think I'm going to ask if my PCP will take over the script. He probably won't put me under this microscope.

Are you sure is says pg/ml? That's like non existent like 8.3? That's female level brother is you shbg like 120?

Yes pg/ml. The medical portal that posts my results lists the Free Testosterone Normal Range as 7.2 - 24 pg/ml. They didn't run SHBG so I can only guess it's high.
I'm really trying to find ways to raise this

I tried for so long to get a legit TRT script. I didn't want an online clinic ($$). It was only coincidence at the urologist I saw signs for TRT. He was really friendly about it and after two blood draws had my insurance approved and going. Now.... I'm beyond frustrated.
It's like they're ignoring my Free and basing the entire thing on the published total regardless of how I feel. If I can't find someone to transfer to I'm hoping just my PCP will do it as he won't micro manage it.

How long have you been on this?

They are still dialing it in. Be more patient. You start fucking with other shit and it muddies the picture.

Edit out the personal info on your bloodwork report and post it. We can’t get the full picture until you do that.

The other blood markers weren't run this time. I understand dialing in. I just got frustrated when the ordered 3x /week. My biggest concern right now is this low Free Testosterone. Idk what can be done? They want my total a little bit lower which I worry will drive free way down.
I assume it's better to have higher free regardless of total?

PS: it's been 3 months now. Blood each month. They only ran a full panel the first time, now just the testosterone and estradiol.
